A national child protection database should be established to prevent further deaths at the hands of violent and neglectful parents, South Australia's deputy coroner says.
Anthony Schapel made the recommendation on Thursday at the conclusion of an inquest into the death of four-month-old baby Ebony, who died from horrific injuries at the hands of her father in Adelaide in 2011.
The inquest heard that Ebony's father, who has been jailed after pleading guilty to criminal neglect, had a history of violence against children in his home state of NSW.
But Families SA and SA Police failed to properly investigate his background even after he admitted to causing Ebony to break her leg while under the influence of drugs.
A database will help but in this case, the people who should have done their job properly failed this child - no database will help if people dont bother to look
